CarePlus Health Plans, Inc. Medicare Advantage approved HMO benefit plans offer broad coverage. Depending on the plan, you can choose from a plan with ample prescription coverage, to one with rich dental benefits.

How these plans work

CarePlus Health Plans, Inc. Medicare Advantage approved HMO benefit plans let you choose your own primary care doctor from an extensive list of physicians. Together, you and your doctor coordinate all of your care. You even have the option to change your Primary Care Physician at any time. Your Primary Care Physician, called a PCP, is your partner in health and will send you to specialists or hospitals when you need specialized care.

CarePlus Health Plans, Inc. Medicare Advantage approved HMO benefit plans cover all the benefits of Original Medicare and much more, including:

• Medically necessary doctor visits
• Hospitalization coverage
• Worldwide coverage for Emergency

Please remember that you must use CarePlus' network providers.
